Periimplantitis
Bei Periimplantitis finden sich:
Bluten und/oder Suppuration auf Sondierung,
Zunahme der ST im Vergleich zu Voruntersuchungen und
Knochenverlust über die Knochenveränderungen hinaus, die durch initiale Knochenremodellation entstehen. Dabei dient ein Röntgenbild, das ein Jahr nach Belastung des Implantates angefertigt wurde, als Referenz.
Sollten keine Unterlagen von vorhergegangenen Untersuchungen zum Vergleich vorliegen, kann eine pragmatische Diagnose der Periimplantitis auf folgenden Kriterien basieren:
Bluten und/oder Suppuration auf Sondierung,
ST ≥ 6 mm und
ein Knochenniveau ≥ 3 mm apikal des am weitesten koronal gelegenen Teils des intraossären Implantatanteils16.
Periimplantäre Weich- und Hartgewebedefekte
Der Heilungsprozess nach Zahnextraktion führt zu reduzierten Dimensionen des Alveolarfortsatzes mit resultierenden Weich- und Hartgewebedefekten. Ausgeprägtere Defekte treten an Stellen auf, an denen es zuvor zu parodontalem Attachmentverlust, endodontischen Infektionen, Längsfrakturen der Zahnwurzeln und/oder zusätzlichem Trauma während der Extraktion gekommen ist. Dünne bukkale Knochenlamellen, Verletzungen, die Belüftung der Kieferhöhle, Medikation, systemische Erkrankungen, die den Knochenstoffwechsel beeinflussen, sowie Druck von Prothesen können die Situation verschärfen und die Insertion von Zahnimplantaten erschweren16.
